Important Information for My Patients of Clifton Park Family Medicine
With the retirement of my colleague Dr. Shelley Justa, Clifton Park Family Medicine has arranged to consolidate with our neighboring CCP family practice in Clifton Park. Effective January 9, our office will become a part of Community Care Family Medicine and remain in its current location.
While I have thoroughly enjoyed my time here, I will relocate to another primary care practice in Community Care Physicians (CCP), CapitalCare Family Practice Clifton Park. I would be happy to continue to care for you at my new practice in Clifton Park – the address is 939 Route 146, Suite 700, Clifton Park, NY 12065. You can also choose to remain with your current practice here in Clifton Park at Community Care Family Medicine.
I have sent a communication to all of my patients via mail and email. Please view a copy of the communication here. If you need an appointment with me before January 9th, please continue to contact Clifton Park Family Medicine. For appointments after January 9, please either call my new office, CapitalCare Family Practice Clifton Park, at (518) 383-0891 or call CCFM at (518) 371-9355 to establish with a new primary care provider in Clifton Park at Community Care Family Medicine. Whichever office you choose, as practices of CCP, you will not need to transfer records. I am confident you will continue to receive the care and service you deserve at any Community Care location you choose.
Rundi Constant is a board-certified family nurse practitioner and a member of the New York Nurse Practitioner Association. He received his Master of Science in nursing from Mount Saint Mary College in Newburgh, NY. Rundi began his studies at CUNY College of Staten Island in Staten Island, NY, earning his Bachelor of Science in nursing. He has a particular interest in preventative care and health promotion.
Rundi comes from a family of healthcare providers. Growing up, he always wanted to continue in his relatives’ footsteps. Becoming a registered nurse and finally, a nurse practitioner was always his passion. What Rundi loves most about his job is seeing his patients’ enthusiasm toward achieving the best version of themselves. He believes treating patients holistically is most important. Rundi strives to provide patients with the highest level of care possible, emphasizing education and promoting healthy behaviors. His care philosophy as a provider is always to remember that life is the most precious thing ever given to us. What brings him the most satisfaction professionally is seeing his patients achieve their health goals with his guidance and teachings.
Rundi is originally from New York City but moved to the Capital Region, specifically Glens Falls, for the family atmosphere. He enjoys hiking, biking, and traveling. Rundi also loves spending time with family and friends.
