Important Information for My Patients of Clifton Park Family Medicine

With the retirement of my colleague Dr. Shelley Justa, Clifton Park Family Medicine has arranged to consolidate with our neighboring CCP family practice in Clifton Park. Effective January 9, our office will become a part of Community Care Family Medicine and remain in its current location. You do not need to transfer care. Nothing changes. I have sent a communication to all of my patients via mail and email. Please view a copy of the communication here and contact Community Care Family Medicine at (518) 371-9355 for appointments after January 9.
